When you submit the qualification form, we collect:

- Your clinic name, clinic type, and website (optional)
- Your clinic’s monthly revenue range (you may choose “Prefer not to say”)
- Whether you agreed to receive SMS messages from us
Along with the form, we automatically record the page address you submitted from and your browser’s user agent (browser and device type). We do not use advertising cookies or third-party analytics on this website.
This form is intended for businesses. Please do not submit patient health information or any other sensitive personal data.
How we use your information
- To review whether your clinic is a fit for our services
- To contact you about your submission and our services by email, phone, or, if you opted in, SMS
- To keep records of inquiries and improve our website
We do not sell or rent your personal information.
SMS messaging

Text messaging originator opt-in data and consent will not be shared with any third parties.
